New Yorkers with disabilities and/or seniors can choose to participate in these New York State Home and Community-Based Medicaid Waiver Programs and live safely in their community.

WAIVER PROGRAM ELIGIBILITY
Individuals seeking to participate in the NYS Home & Community-Based Medicaid Waiver Program must:
- Be at least 18 years of age.
- Be an active New York State Medicaid recipient.
- Have medical documentation of brain injury, have a physical, medical or cognitive diagnosis.
- Be eligible for skilled nursing facility level of care.
- Choose to live in the community rather than a skilled nursing facility.
- Be able to identify a safe living arrangement that will meet their needs.
- Be capable of living in the community with appropriate supportive services.
What services are available under the waiver programs?
- Service Coordination
- A coordinator is chosen by the waiver participant and/or guardian to work with them in identifying and coordinating support and services and developing a written service plan.
- Home and Community Support Staff
- Similar to Home Care services, this service also provides oversight, cognitive cueing and support to enable the waiver participant to reside safely within the home and community environments.
- Independent Living Skills Training
- One-to-one training to improve or maintain the ability to live as independently as possible, focusing on essential community living skills.
- Community Integration Counseling
- Counseling service for those who are coping with changes in their abilities and skills, as it relates to their disability/diagnosis.
- Structure Day Program
- Community based social model program to provide socialization and skill building opportunities.
- Positive Behavioral Interventions and Supports (PBIS)
- A community-based provision of direct assistance to individuals who may present with challenging behaviors.
- Social Transportation
- Provides the means to access and use non-medical services in the community and improve integration in the community.
- Environmental Modifications
- Provides adaptations to residences to promote independence and to ensure safety.
- Assistive Technology
- Equipment that will improve independence, decrease reliance on staff and be a cost-effective aid for community integration.
- Community Transitional Services
- Assistance in transitioning from a nursing home back to the community, including the cost of moving, essential furnishings, deposits for utilities, security deposits or one-time cleaning services prior to occupancy.
- Other services include:
- Respite services, substance abuse programs, congregate & home delivered meals or community transitional services.
